US 121 was an original US Highway, arriving in North Carolina in late 1926. It was added to the entirety of NC 66. It began at US 70/NC 10 (now just Main St) Lexington then headed up today's NC 8, then Old US 52 to Welcome (used Old Winston Rd) and Midway. US 121 then headed east on Gum Tree Rd, then north on Old NC 109 and Old Lexington Rd. US 121 used Waughtown Rd St, then Main St north into Winston-Salem.
US 121 followed Patterson Ave north to Stanleyville, then followed today's NC 66 to Rural Hall, then picked up "Old US 52" to King, where old 66 used E. Dalton Rd and W. Dalton Rd out to Dalton. US 121 then likely used Dalton Loop Rd and old US 52 to Pinnacle. US 121 then followed VFW Rd, Old 52 Rd and Old Winston Rds from Pinnacle to Pilot Mtn. US 121 used Main St through Pilot Mtn. US 121 then used Old US 52 and US 52 Business to Mt. Airy (except US 121 used Main St all the way through downtown), turning left with current US 52 Business then used Old Fancy Gap Rd to the Virginia State Line where US 52 now crosses over. In 1928 or 1929, US 121 was rerouted south of Winston-Salem to use today's Spur St south which becomes Old US 52. The old route to the east remained a piece of NC 77 and another part became a piece of NC 109 for a brief time.
In late 1934, US 121 was renumbered as part of an expanded US 52.
