US 221-A was born in 1941 as a replacement for mainline US 221 from Rutherfordton through Forrest City, Caroleen, and Cliffside before ducking into South Carolina to meet US 221 in Chesnee, SC.

Between 1958-62, US 221-A was removed from SR 1993 Old US 221-A near the state line.
In Aug 1971 (NCDOT), US 221-A was removed from Old Main St and abandoned segment over to Boiling Springs Rd in Cliffside.
