Creation: SC 165 appeared in 1940 as new primary routing, running from US 17 Ravenel south to Yonges Island. This is the only SC 165.
Adjustments: In 1953, SC 165 was extended north over S-10-83 and S-18-52 to Cooks Crossroads, then replaced SC 61 through Summerville to end at US 78 northwest of town.

In 2000, SC 165 was rerouted to avoid downtown Summerville by using Gum St to US 78 and new construction to US 17 ALT near I-26. The old route became S-18-64, US 17 ALT, and S-18-65 Richardson St.

Between 1995-2005, SC 165 was straightened out in Hollywood, eliminating a brief SC 162 overlay and leaving behind Town Council Rd.
Improvements: SC 165 was paved upon inception.
A small piece of SC 165 above US 17 ALT was multilane by the early 80's.
By the late 80's, SC 165 was multilane around the Trolley Rd (S-18-199) area.
The 2005 reroute around Summerville to US 17 ALT near I-26 is multilane.
Comment: SC 165 below SC 61 is a dark, scary ride through the swamp, especially in the middle of the night.
SC 165 is barely posted south of Hollywood.
