Creation: SC 381 appeared in 1929 as new primary routing, running from SC 30 (now US 15-401) McColl south and east through Clio and Blenheim before heading west back to end at SC 30 (now US 15-401) opposite of where SC 912 ends today. This is the only SC 381.
Adjustments: In 1933, SC 381 south was truncated to end at SC 38 Blenheim. The old route west to SC 30 today is part S-35-50 and S-35-57.
In 1934 or 1935, SC 381 was removed from today's S-35-497 between Clio and McColl.
In 1936 or 1937, SC 381 north was extended as new primary routing to end at the North Carolina line.

The rest of SC 381 from Blenheim to NC was paved in 1938.

Comment: SC 381 can be used as a short-cut from I-74/US 74 to I-95 while avoiding Bennettsville.
