Creation: SC 381 appeared in 1929 as new primary routing, running from SC 30 (now US 15-401) McColl south and east through Clio and Blenheim before heading west back to end at SC 30 (now US 15-401) opposite of where SC 912 ends today. This is the only SC 381.
Adjustments: In 1932, SC 381 south was truncated to end at SC 38 Blenheim by Act of SC Legislature in a mileage swap for SC 96. The old route west to SC 30 today is part S-35-50 and S-35-57.
In 1934 or 1935, SC 381 was removed from today's S-35-497 between Clio and McColl.
In 1936 or 1937, SC 381 north was extended as new primary routing to end at the North Carolina line.

In 1940, SC 381 south was extended over the 1929-32 SC 381 routing to SC 382 (now S-35-18) near Scott.
In 1947, SC 381 south was again truncated to SC 38 Blenheim, leaving behind S-35-25 (now S-35-50).
