VA 15 #1: Began in January 1918 as an original state route defined by the general assembly: Route 15: Woodstock, Calvary, West Virginia Line In 1923, VA 15 was renumbered as VA 331. Today this is a 4wd trail from the WV Line south to Forest Rd 252 (the gravel road that currently runs from old WV 59 over to SR 675 was NOT VA 15); Forest Rd 252 south to SR 691; SR 691 to Liberty Furnace; SR 717 east to SR 675; SR 675 east to Columbia Furnace; SR 623 and SR 768 over to VA 42; VA 42 east to the I-81 area; Fairground Rd to US 11 in Woodstock

VA 15 #2: Appeared in 1923 as a renumbering of VA 12Z, running from VA 10 (now F044) Fort Chiswell south via Hillsville to the North Carolina line below Cana.
In late 1926, US 121 was assigned to the entirety of VA 15.
In June 1931 (CTB), VA 15 was extended north as new primary routing to Max Meadows.

Around 1931 or 1932, VA 15 was removed from Old Jacksons Ferry Rd and put on the original New River Bridge in this location.
In July 1933, VA 15 was dropped from US 121. Fort Chiswell to Max Meadows became VA 121.
The route of VA 15 today is: VA 121 south; abandoned alignments near I-81; US 52; SR 632/738 loop; US 52; Galena Church Rd; US 52; SR 719 Old 52 Rd; VA 69; US 52 to Hillsville except Creek Ln; US 52 to NC except SR 906 Kinglet Ln, Wisler Rd, SR 892, SR 841 Mainline Rd.
