Creation: SC 391 appeared in 1923 as new primary routing, running essentially the way it does now. This is the only SC 391.
Adjustments: In 1936 or 1937, SC 391 was extended north along what had been US 76 and SC 2 through Prosperity to SC 391's current north ending.
In 1939, SC 391 was extended south as new primary routing to SC 215 (now SC 4-302) Couchton.
In 1940, SC 391 south was rerouted to follow SC 39 south for 6 miles before angling southwest to the same endpoint at SC 215. The old route is now S-2-49 and S-2-264.
In 1947, SC 391 south was truncated back to Jones Crossroads, leaving behind SC 39 and S-2-21.

Comment: SC 391 meets US 378 at one of the few traffic circles in South Carolina. This may be the most rural traffic circle in the eastern U.S.
