The Roads:

The oldest active US 64-A was born in Nov 1978 (NCDOT) as a replacement for NC 98 (really!) and is the 12th US 64-A. It runs between US 64/NC 98/NC 231 west of Spring Hope and ends at US 64/64 Business Nashville.
US 64-A follows original NC 90 except for Quiet Waters Rd.
This was erroneously labeled as US 64 Bus on the 1979-80 Official but is shown as US 64-A on the 1980 Nash County Map.

The second oldest current US 64-A was created about 1987 as a renumberinbg of mainline US 64 and is the 13th US 64-A. It runs from US 64/64 Business Rocky Mount to US 64-258/NC 111-122 Tarboro.
US 64-A follows NC 90 except at Tarboro it follows the 1960s bypass of Tarboro.

The newest active US 64-A was created about 1996 as a renumbering of mainline US 64 and is the 15th US 64-A. It initially ran from US 64-258/NC 111-122 Princeville east to US 64 (using Roberson School Rd) east of Bethel.
About 2000, US 64-A was extended east replacing US 13-64 to NC 903 Robersonville.
In 2001 or 2002, US 64-A was extended over more US 13-64 over to US 17/NC 125 along the original Williamston Bypass.
In 2004, US 64-A was extended over US 64 to its current endpoint of US 13-17-64 east of Williamston.

Now a chronological look at decommissioned US 64-A routes:

The 1st US 64-A appears to go back to the great renumbering of late 1934. It ran through Rocky Mount on Thomas Ave while mainline US 64 used Sunset Ave, Raleigh St and Tarboro St. This routing is shown as unlabeled primary route in the 1932 Texaco inset of Rocky Mount and my guess was that this was NC 90-A first.
This US 64-A became part of mainline US 64 between 1945-49. Today it is the westbound split of US 64 Business.

The 2nd US 64-A appeared Jan 1, 1941 (per July 1940 AASHO letter). It began at US 64 Bat Cave and duplexed with US 74 east to Ruth, then replaced NC 181 northeast to meet US 64-70 (now Union St) Morganton.
In 1948, US 64-A became mainline US 64.

The 3rd US 64-A appeared about 1941, replacing US 64 through Franklinville over to Ramseur.
About 1957, US 64-A was downgraded to SR 2235 Andrew Hunter Rd and part of NC 22.

The 4th US 64-A was created in 1943 or 1944 as a new primary routing in Brevard, running along Caldwell St.
In 1960, US 64-A was renumbered as US 64 Business which was later downgraded but became US 64 Business again in the late 2000s.

The 5th US 64-A was created about 1949 as a renumbering of mainline US 64 through Plymouth. An October 1960 letter to AASHO explicitly said US 64-A was renumbered to US 64 Business through Plymouth. It is unclear if US 64-A went back to the bypass opening or was added later. No county map shows 64-A or 64 Business. The route was today's SR 1335 Wilson St/3rd St; SR 1357 Washington St; SR 1325 Main St.

The 6th US 64-A was created about 1952 as a renumbering of mainline US 64 through Siler City. It ran along 3rd Ave through town.
US 64-A was downgraded to SR 1107 about 1957.

It appears the 7th US 64-A was born in Salisbury in 1953 using US 220 from the new US 64 north to Salisbury St then east over former US 64 out to the new US 64. The 1953 Randolph County map shows no route number and the 1957 Randolph County map shows NC 902 (now NC 42) on Salisbury west of the NC 42 turnoff and secondary routing (now SR 2237) east of there. However, State Officials 1953-56 imply a US 64-A was present.

The 8th US 64-A was born about 1954 as a replacement for mainline US 64 through Williamston.
US 64-A used Main St to downtown, then Haughton St and Sycamore St (now Jamesville Rd) back out to US 64.
In 1960, US 64-A became US 64 Business. Today Main St and Jamesville Rd are SR 1445 and SR 1446 while Haughton is part of US 17 Business.

The 9th US 64-A was born about 1954 as a renumbering of mainline US 64 through Statesville.
US 64-A began at Westminster Dr and ended at the I-40 interchange east of I-77.
US 64-A was renumbered as US 64 Business in 1960. Today it is again mainline US 64 except the westernmost piece is part of US 70.

The 10th US 64-A was born in 1955 as a renumbering of mainline US 64 through Roper along SR 1122 Buncombe St.
US 64-A became US 64 Business in 1960. An October 1960 letter to AASHO explicitly said US 64-A was renumbered to US 64 Business through Roper. It is unclear if US 64-A went back to the bypass opening or was added later. No county map shows 64-A or 64 Business.

The 11th US 64-A was born about 1960 as the connector from the temporary end of I-40 to US 64-70 at Hildebran. It is shown but not labeled on the 1962 Burke County map, but explicitly labeled from at least 1968-72 on the county maps.
When I-40 was extended east in 1976, US 64-A was downgraded to SR 1890.

The 14th US 64-A was born in late 1987 or early 1988 as a renumbering of US 64 from Morganton to Statesville as a 100% duplex with US 70.
In July 1988 (NCDOT), the US 64-A designation was recognized as erroneous and was dropped (see the last page of this NCDOT document). This never appeared on an official map but has been shown on Rand McN atlases from that time through at least 2014.

NCDOT applied to AASHTO for the creation of a 16th US 64-A to run along the vacated US 64 routing from Murphy to NC 141. This request was rejected and the road became secondary instead.
